Bioidentical Hormones Explained: What They Are, What the Evidence Says, and What to Know

What Does "Bioidentical" Actually Mean?

At its core, bioidentical means molecularly identical to the hormones your own body produces. A bioidentical hormone has the exact same chemical structure — the same arrangement of atoms, the same molecular geometry — as the endogenous hormone it's designed to replace.

This matters because hormones work by binding to specific receptors, and the fit between a hormone and its receptor is extremely precise. A hormone with a slightly different molecular structure (a synthetic analog or a non-human form) may bind to receptors differently, activate different downstream pathways, and produce different — sometimes unwanted — effects compared to the body's own hormone.

Bioidentical vs. Synthetic: Why the Distinction Matters

The most clinically significant comparison is between micronized progesterone (bioidentical) and synthetic progestins like medroxyprogesterone acetate (MPA). The WHI study used MPA, not micronized progesterone — and this distinction may explain much of the difference in outcomes between the WHI and more recent studies using body-identical hormones.

Research suggests that synthetic progestins, unlike micronized progesterone, may have adverse effects on breast tissue, mood, and cardiovascular parameters. The ESTHER study (2007) and the French E3N cohort study both found significantly lower breast cancer risk with transdermal estradiol plus micronized progesterone compared to regimens using synthetic progestins. These differences are real, they're clinically meaningful, and they're why the type of progesterone you use matters enormously.

A Word on Saliva Testing

Some providers who specialize in compounded bioidentical hormones rely heavily on saliva testing to measure hormone levels. This approach has significant limitations. Salivary hormone levels are highly variable, affected by food, hydration, time of day, and collection technique. They don't correlate reliably with serum levels, and there are no standardized reference ranges. Major hormone societies — including the Endocrine Society and the Menopause Society — do not recommend saliva testing as a primary method for monitoring HRT.
