But there's one more hormonal player that rarely gets the spotlight it deserves: DHEA — dehydroepiandrosterone. This age-related decline in adrenal DHEA production is sometimes called adrenopause — and unlike the more dramatic hormonal shifts of menopause, it happens gradually and silently over decades.
Because DHEA contributes to the production of estrogen and testosterone, its decline compounds the hormonal deficits of menopause. This is a significant milestone: Intrarosa is the first FDA-approved DHEA product for women, and it provides strong clinical validation of DHEA's role in vaginal health.
Unlike estrogen-containing vaginal preparations, Intrarosa works by supplying DHEA locally, which is then converted to both estrogen and testosterone within vaginal tissue. Serum hormone levels remain low because conversion is local. This makes it a compelling option for women who cannot or prefer not to use systemic or local estrogen.
Oral vs. Injectable DHEA and Testing
Oral DHEA is available over the counter in the United States as a dietary supplement, making it one of the most accessible hormonal interventions available. Typical supplementation doses range from 10–50mg daily for women, with lower doses often preferred to avoid conversion to androgens that could cause side effects at higher levels.
Injectable DHEA preparations are also available through compounding pharmacies and offer higher and more consistent bioavailability than oral forms, which are subject to variable first-pass metabolism.
Before supplementing, baseline DHEA-S levels (the sulfated, more stable storage form measured in blood) should be established. Monitoring levels during supplementation allows for appropriate dose adjustment. DHEA converts to both estrogen and testosterone, so women who are sensitive to androgens should use caution with higher doses and monitor for signs of excess (acne, oiliness, excess hair growth).
