Menopause

That's it. Menopause is technically one day. Everything before that day (during the transition) is perimenopause. Everything after is postmenopause.
Perimenopause
Perimenopause is the transition phase leading up to menopause. It typically begins 8–10 years before the final menstrual period — meaning for most women, somewhere between ages 40 and 45.
During perimenopause, hormone levels don't simply decline steadily. They fluctuate erratically — sometimes surging to highs, sometimes crashing to lows. This volatility is why perimenopausal symptoms can feel so unpredictable and confusing.

Why Lab Values Mislead During Perimenopause
Here's a critical clinical point: a single hormone test during perimenopause can be misleading.
Because estrogen fluctuates so dramatically during the transition, you can test your estradiol on a day it's surging and get a "normal" premenopausal reading — even if you're deeply in perimenopause. A week later, the same test might show a postmenopausal level.
FSH has the same problem. An elevated FSH suggests the pituitary is working hard to stimulate ovaries that aren't responding well — a perimenopause marker. But FSH surges with each attempted ovulatory cycle, so it fluctuates too.
This is why pattern and timing matter more than a single snapshot, and why symptom-based assessment is an essential component of perimenopausal evaluation — not just labs.
For women who are still cycling, testing in the early follicular phase (days 2–5 of the cycle) gives the most stable picture.

Women in Perimenopause
