Best Supplements for Menopause: What the Evidence Actually Shows

Walk into any health food store and you'll find an entire wall dedicated to "menopause support" supplements. Bold claims. Reassuring packaging. Prices that suggest they've been hand-blessed by an herbalist. But which of these products actually have meaningful clinical evidence? And which are mostly wishful thinking in capsule form?

Let's cut through the noise. Below is an honest, evidence-based look at the supplements that are worth considering during perimenopause and menopause — and the important limitations of what they can (and cannot) do.

The Evidence-Backed Essentials

Magnesium glycinate (200–400 mg at night) is one of the most clinically useful supplements for perimenopausal women, and one of the most commonly deficient minerals in the modern diet. It promotes deeper, more restful sleep by supporting GABA activity in the brain — the calming neurotransmitter that helps quiet an overactive nervous system. Studies show magnesium supplementation improves sleep onset, sleep duration, and subjective sleep quality. It also reduces anxiety, supports bone mineral density, helps regulate blood sugar, and eases muscle cramps. The glycinate form is better absorbed and gentler on the digestive system than magnesium oxide.

Herbal Supplements With Actual Evidence

Black cohosh (Actaea racemosa) is probably the most studied herbal remedy for menopause. Multiple meta-analyses suggest it provides modest reduction in hot flash frequency and severity — roughly a 25–30% reduction compared to placebo in some studies, though results are inconsistent. It appears to work through serotonin and dopamine pathways rather than as a phytoestrogen, which is why it's sometimes recommended for women who can't use estrogen. Side effects are generally mild (occasional GI upset, headache); rare cases of liver toxicity have been reported, so it should not be used by women with liver disease. Standardized extracts (20 mg isopropanolic extract twice daily) are the best-studied form.

Ashwagandha (Withania somnifera, 300–600 mg of KSM-66 extract daily) is an adaptogenic herb with a growing body of clinical evidence for stress reduction, cortisol regulation, and thyroid support. The Important Caveat: Supplements Don't Replace Hormone Therapy
