Testosterone. Most women think of it as a male hormone — something that gives men deep voices and aggressive tendencies. But here's what your doctor may never have told you: testosterone is the most abundant biologically active hormone in a woman's body. Not estrogen. Testosterone.
Women produce testosterone throughout their lives — in the ovaries and adrenal glands — and they depend on it for far more than most people realize. When levels decline with age and through the menopause transition, the effects can be wide-ranging and deeply disruptive. Yet testosterone for women remains one of the most underdiagnosed and undertreated deficiencies in women's health. That needs to change.
What Testosterone Does in Women's Bodies
Testosterone plays a critical role in multiple systems throughout a woman's body:
- Libido and sexual function — Testosterone is the primary hormone driving sexual desire in women. Low testosterone is one of the leading causes of hypoactive sexual desire disorder (HSDD) — the persistent, distressing loss of libido. - Energy and vitality — Testosterone supports mitochondrial function, red blood cell production, and overall metabolic energy. Women with low testosterone often describe a bone-deep fatigue that no amount of sleep seems to fix.
- Muscle mass and strength — Testosterone stimulates muscle protein synthesis and helps maintain lean body mass. Its decline contributes to the loss of muscle tone and strength many women notice in their 40s and 50s.
- Mood and motivation — Testosterone has direct effects on dopamine pathways and is closely linked to drive, confidence, and emotional resilience. Low levels are associated with flat affect, low motivation, and depression that doesn't respond well to antidepressants.

How and When Testosterone Declines in Women
Testosterone levels in women peak in the mid-20s and then begin a gradual, decades-long decline. By the time a woman reaches her 40s, her testosterone levels may be half what they were at their peak. The menopause transition accelerates this decline — both as a consequence of ovarian changes and because rising SHBG (sex hormone-binding globulin) during hormonal fluctuation further reduces the amount of free (biologically active) testosterone available.
Women who undergo surgical menopause (bilateral oophorectomy — removal of both ovaries) experience an even more abrupt and significant testosterone drop, since the ovaries are responsible for roughly 25–50% of circulating testosterone. These women often notice a dramatic and sudden change in libido, energy, and mood that is frequently undertreated.
The 2019 Global Consensus Statement
In 2019, a landmark international consensus statement on testosterone therapy for women was published in major medical journals including Menopause, the Journal of Clinical Endocrinology & Metabolism, and others. Endorsed by 12 major medical societies worldwide, it concluded:
- There is Level 1 evidence (the highest level) supporting the use of testosterone for the treatment of HSDD in postmenopausal women
- Testosterone therapy, when used to achieve physiological female levels, does not increase cardiovascular risk, is not associated with increased breast cancer risk, and does not cause liver damage
- Testosterone should be considered a standard component of women's HRT, not an experimental or fringe treatment
This consensus represented a turning point — the clearest scientific endorsement yet that testosterone therapy for women is safe, effective, and long overdue in mainstream medicine.
Delivery Methods for Women
Because no testosterone product is currently FDA-approved specifically for women in the United States, women's testosterone therapy is typically prescribed off-label or via compounding pharmacy. Options include:
- Compounded testosterone creams or gels — Applied to the inner wrist, labia, or thigh in very small doses (typically 0.5–2mg daily for women, far less than male doses). This is the most common approach and generally well-tolerated.
- Low-dose injectable testosterone — Compounded testosterone cypionate or propionate injected subcutaneously or intramuscularly at doses appropriate for women. Provides stable levels and excellent bioavailability.
- Testosterone pellets — Subcutaneous implants that release testosterone slowly over 3–6 months. Effective but not adjustable once inserted.
- Off-label use of male products at female doses — Some providers use commercially available testosterone gels at a fraction of the male dose. Requires careful calculation and monitoring.
Safety at Female Doses: What You Need to Know
The safety concerns that sometimes surround testosterone therapy for women are almost entirely dose-related. At doses appropriate for achieving female physiological levels (free testosterone in the normal female range), testosterone therapy has an excellent safety profile. Concerns about virilization — voice deepening, clitoral enlargement, excess hair growth — arise primarily from doses that exceed normal female ranges, which can happen when dosing protocols are not properly calibrated or monitored.
Appropriate monitoring includes baseline and follow-up total and free testosterone, SHBG, and hematocrit levels. When testosterone is prescribed and monitored carefully, side effects are uncommon and generally reversible by dose reduction.
